RESPONSIBILITIES
  • Provides access and referral to concrete services on behalf of residents.
  • Carrying caseload and performing Case Management duties.
  • Communicate with family, collaterals, and other agency staff on behalf of residents as a part of service planning.
  • Conduct intake screenings.
  • On call during off-hours and weekends, available for consultation on an as needed basis.
  • Participate in Utilization Review, staff rounds, and discharge planning.
  • Professional visits and off-site screenings at other Agencies.
  • Maintain appropriate documents, records, and statistics and write reports in an organized, timely, and accurate manner.
  • Accompany clients to outside appointments, hospital/clinic emergencies, field trips as necessary.
  • Conduct various structured and therapeutic activity groups especially communication skill personal awareness, group therapy, men/women group, task activity therapy; will maintain short/long term goals.
QUALIFICATIONS
Bachelors Degree in Social Work or a High School Diploma and at least 5 years experience working with homeless and mentally disabled individuals.
REQUIREMENTS
  • Good verbal and written communication skills a must. 
  • Bilingual English/Spanish Preferred.

SUMMARY

Provides basic case management, advocacy and emotional support to adolescents and adults engaging in or at risk of engaging in prostitution, intravenous drug use and other behaviors which put them at risk for HIV/AIDS, sexually transmitted diseases and pregnancy. Determines clients eligibility for case management and non case management. Responsible for filling out, discussing and reviewing the initial intake packet with all office and outreach clients. Establishes informal/formal linkages with service providers delivering necessary services to our target population. Provides individual counseling and pre and post HIV test counseling. Arranges for the provision of health and concrete emergency services. Maintains current case files and performs follow-up on all individuals serviced by the Program. Organizes and facilitates weekly case conferences with Outreach Staff and establishes and facilitates client discussion/support groups.

QUALIFICATIONS

Requires BA in Social Work or equivalent. Requires at least 3 years experience in providing case management services to multi-problem youth and disenfranchised population. Must be sensitive to cross cultural differences and issues related to sexual identity. Bilingual Spanish/English preferred but not required.

Salary details: 33,000 - 35,000

For the past 32 years, Women Aware has been saving the lives of battered women and their children. Supported by a budget of $1.75 million from a variety of sources, we offer services in three locations including a 24-hour hotline, emergency shelter, legal advocacy, community education, support groups, counseling, and permanent supportive housing. All services are free and confidential. We are a state and national leader in the field - selected by the NJ Department of Children and Families to expand its Domestic Violence Liaison Program and launching the federal Jessica Gonzalez Special Victim Assistance Program for the State of New Jersey. These, as well as all of our programs and services demonstrate that we are moving ever closer to ending domestic violence in the lives of women, children and men; changing societal attitudes and institutions that promote and condone violence, and empowering survivors to live free of violence.

We are seeking a PT Case Manager (24 hours per week) for our PALS program, working with adult victims of domestic violence, needed for non-residential counseling program. Excellent verbal, written, and crisis-intervention skills. Bilingual Spanish and Bachelor degree required; DV experience and Master's degree preferred.

DESCRIPTION

New Providence Women's Shelter is located on East 45th Street in Manhattan and serves two groups of clients: women who are diagnosed as mentally ill or mentally ill and substance abusers, and women who are solely substance abusers. At New Providence, our clients learn how to cope with their disabilities and recreate their lives, eventually moving on to their own housing and jobs. The New Providence Shelter staff's goal is to stabilize, treat and offer rehabilitation to women who will ultimately be helped to move on to long-term housing.

We are currently seeking a Case Manager to join our interdisciplinary team.

The case manager will provide counseling and case management services aimed at helping the residents participate in treatment plan, develop living skills appropriate to their disability and prepare to live independently. The case manager will be responsible for initially interviewing each of his/her clients individually, assessing the client's needs and developing and implementing a client service plan. The case manager will also assist in obtaining financial entitlements, housing and other concrete services as well as provide group facilitation, as needed. Additionally, case managers are expected to escort clients to appointments, as necessary.

The case manager will carry a caseload of approximately 15-20 clients.
Work shift may float during week and one late night (11:00am-7:00pm) is required.

SKILLS

BA/BS in related human service discipline with experience working with Mentally Ill and MICA clients preferred. Bi-lingual (English/Spanish) required. Must have knowledge of Assisted Outpatient Treatment process.

JOB SUMMARY


The purpose of the position is to assure that each Marian House II resident continues improvement through the delivery of mental health and recovery services and oversight of structured activities including regular attendance at work and house management. The incumbent is required to provide individual counseling and case management services to designated residents and to co-facilitate group counseling as directed. Marian House II Case Managers supervise all women who reside at Marian House II sites. Working in conjunction with Marian House staff and other MH II case manager/counselor(s), she coordinates the Marian House II program, counsels and assists each individual woman, and links her with other services as needed.


DUTIES AND RESPONSIBILITIES
  • Assists women as they transition from MH I to MH II.
  • Works closely with each woman's counselor at MH I to insure continuity in the program.
  • Provides close supervision and frequent face to face meetings with residents of MH II during the initial transition Bridge Period.
  • Provides formal case management services monthly with special focus on financial management issues as well as assisting residents in accessing other programs as needed.
  • Provides individual clinical counseling twice a month and as necessary.
  • Counsels residents regarding recovery, physical and mental health, employment, education, money matters, life skills, relationships, goals, and plans for the future.
  • Provides a stable presence at MH II sites, encouraging community life and frequently interacting informally with the women.
  • Is available for unexpected issues and circumstances as they arise among the women.
  • Plans and guides a required monthly meeting of residents, allowing opportunities for sharing and for the airing of residents' concerns.
  • Plans and co-facilitates therapeutic group counseling for MH II residents twice a month. Coordinates a monthly relapse prevention group and provides oversight, as necessary.
  • Supervises the care and cleaning of the physical plant.
  • Assists residents as they prepare to move beyond MH II with developing appropriate housing plans.
  • Maintains detailed case records and develops treatment plans in conjunction with the client as assigned.
  • Make presentations in the community on topics related to mental health, trauma, & addiction, as directed.
  • Attends staff, clinical, house, and other meetings as requested.
  • Takes advantage of workshops and conferences that focus on homelessness, addiction, and other topics related to the problems experienced by Marian House women.
  • Cooperate in the agency's team approach to service provision by assuming responsibilities for program operation and house maintenance when needed. This will occasionally require presence outside regular business days and office hours.
QUALIFICATIONS

  • MS in social work or counseling psychology with 2 – 3 years experience in case management and counseling field preferred
  • Current licensure as LCPC or LCSW-C only
  • Excellent interpersonal skills
  • Competence in verbal and written communication
  • Ability to organize, to keep records and manage a workload efficiently.
  • Computer literacy and proficiency in Microsoft applications
  • Background and ability to understand, appreciate, and carry out the Marian House mission; interpersonal skills to engage staff and volunteers in doing same.
