Women's Care Navigator
Friendly, warm, patient care advocate wanted to join our multidisciplinary team caring for women living with HIV at Philadelphia FIGHT. Navigator will meet with women in their homes and the clinic to assess for barriers to care retention and medical adherence. Navigator will assist Lax Women's Team in developing interventions to promote wellness, and will accompany women to specialist visits.
Qualifications include: 3 years experience/cultural competency working with adults who have HIV, mental illness, and substance abuse; work with stigmatized populations building skills for resiliency; strong interpersonal skills with ability to engage in motivational interviewing and provide trauma-informed care from a strengths-based perspective; experience with crisis intervention and de-escalation skills; excellent customer service skills to help resolve case management/systems issues; comfort working in all Philadelphia neighborhoods and use of public transportation. Bachelors preferred.
